1 予約について
- 接種予約はページ内の予約システムよりお願いします。
- 新規登録される方は、①名前以外に②生年月日③電話番号を必ず登録ください。メールアドレスを登録いただくことで前日18時にリマインドメールが届きます。
2 ワクチンについて
- ワクチンの種類はファイザー社のmRNAワクチン（オミクロンXBB.1.5対応）になります。
- 現在はPEG（ポリエチレングリコール）に対するアナフィラキシー既往がある場合を除いて、食事や薬剤へのアレルギー、喘息、アトピー、妊婦・授乳婦、既感染者、悪性腫瘍、自己免疫疾患をお持ちの方々に関しても接種対象としています。
- 公費対象のため、支払いはありません。
3 接種日時
- 現在の接種は、火曜日に最大６名までの接種としています。
4 接種当日
- 当日は必ず①接種券、②「記入した」予診票、③本人確認書類を持参ください。３点を忘れた場合には接種できませんのでご注意下さい。
- 12〜15歳は必ず予診票に保護者の署名が必要になり、16歳以上は必要ありません。詳細はこちらご参照ください。
- 予約時刻に遅れないように余裕を持って来院ください。
- 接種後は最低15分の院内での経過観察時間を要します。（アレルギー既往の内容によっては30分になります）
- ワクチンの保管の性質上キャンセルは原則できません。もし体調不良などや予定変更で急なキャンセルが必要な場合にはできるだけ早くお電話でご連絡いただくようにお願い致します。
5 解熱鎮痛剤について
- 鎮痛剤処方希望の方は、自費処方となりカロナール（アセトアミノフェン）10錠で780円となります。

Information about Covid-19 vaccination
Please make all Covid-19 vaccination reservations from the “Medical Treatment Reservation System” on the website.
We do not accept confirmation of “reservation” / “availability” by phone or visit.
If you are new, please be sure to register (1) your name, (2) your date of birth, and (3) your telephone number.
We apologize for the inconvenience, but we do not provide language support for reservations, so please make your own reservations.
1 About the vaccine
The vaccine is made by Pfizer(XBB.1.5).
The vaccine is covered by public funds and is available free of charge.
The vaccine is intended for people 12 years of age and older.
2 Make a reservation
The vaccination time is only Tuesday. It may change without notice depending on the number of vaccines in stock.
We cannot accept changes in the date and time for both the first and second times. If you need to cancel suddenly due to poor physical condition, please contact us as soon as possible. In this case, all reservations are canceled. Please note that you need to make a reservation again by yourself.
3 Get the vaccine
Please be sure to bring
- vaccination coupon
- “filled-in” pre-vaccination screening Questionnaire
- identity verification documents. If you forget them, you cannot receive the vaccine.
- Please come with plenty of time so as not to be late for the reservation.
- At least 15 minutes of follow-up time in the clinic is required after vaccination. (It may take 30 minutes depending on the content of the history of allergies.)
- 12 to 15 years old must have a parent’s signature on the pre-vaccination form, not 16 years old or older.
