Home-visit medical care

Home-visit medical care is a medical treatment service for those who are recuperating at home and have difficulty going to the hospital, and is planned on a regular basis (once or twice a month).
 
The ability to receive medical care in the comfort of one's own home not only reduces the burden of hospital visits on the patient and family, but also has the advantage of allowing the patient to relax and enjoy the medical care.
In addition, house calls are made by a physician to the patient's home in the event of an emergency, and for those who have a contract with us for home-visit treatment, we have a system in place to make house calls 24 hours a day, 365 days a year on an emergency basis, and even provide physicians care for the patient at the time of death
Please note that we do not make house calls to those who are not under contract for home-visit treatment. We also provide support for those who wish to receive the Maruyama Vaccine.

People who are eligible for home-visit treatment

  • Those who have difficulty visiting hospitals and clinics
  • Those who wish to receive medical treatment at home instead of in the hospital
  • Those who are repeatedly in and out of the hospital and wish to prevent re-hospitalization as much as possible.
  • Those who wish to spend the last days of their lives at home or in a facility.
  • Elderly people who are bedridden at home, etc.

Areas of Home-visit medical care

The clinic serves Meguro-ku, Setagaya-ku, and part of Shibuya-ku, approximately 2 km from the clinic. There is a possibility of expanding the area of our Home-visit medical care services in the future, so please consult with us first.
The clinic also maintains close ties with surrounding medical institutions (Mishuku Hospital, Self Defense Forces Central Hospital, Tokyo Mutual Aid Hospital, Kosei Chuo Hospital, Toho University Medical Center Ohashi Hospital, Tokyo Medical Center, Tokyo Metropolitan Hiroo Hospital, Japanese Red Cross Medical Center, Meguro Hospital, etc.) and can smoothly refer patients when necessary.

Flow Chart to Home-visit medical care

(1) Request for consultation by phone
We welcome any contact from anyone, including the patient, family members, care managers, and the community liaison office.

(2) Hearing
We will ask you a brief history and current situation over the phone or in person.

(3)Explanation and application
After explaining the cost and visiting plan, we will proceed with the contract and decide the date of the initial visit.

(4) Initial visit
We will begin the home visit. During the initial visit, we will take blood samples and perform an electrocardiogram. Please let us know if you have any concerns or questions about the medical treatment.

Home-visit nursing refers to nursing care (medical care or necessary medical assistance) provided by a nurse or other nurse visiting a patient's home under the direction or in collaboration with the attending physician.
We have a home nursing station within our clinic, so why not consider introducing home nursing at the same time as or before introducing home-visit medical care?
